Learning from incidents is a process where an incident is used as a prompt for learning, with the aim of preventing similar events. The process consists of several steps, such as a formal investigation. It is vital that organisations systematically learn from incidents to minimise the chances of tragic events occurring.

My research goals are to: examine how incident information is distributed; investigate how people use lessons learnt from incident investigations in their own work; and understand how this leads to changes in behaviour. At CALRG I will present these goals and the methods I intend to use for research.
